Fazila Ikwaput scored the only goal of the game in the first half of extra time.
The two sides settled for a 3-3 draw in the first leg on Wednesday, which saw late drama deny Uganda a narrow win.
Unlike in the first leg, where a cascade of defensive errors cost the team, the defensive partnership of Aisha Nantongo and Shadia Nankya kept the Rwandan forwards at bay.
Goalkeeper Daisy Nakaziro who replaced injured Ruth Aturo was equally unflappable between the posts.
The two legs were played in Rwanda due to the fact Uganda currently does not have a stadium that passes CAF standards.
Uganda will now face Cameroon in the second qualification round.
